What is Greater Trochanteric Pain Syndrome?

Greater Trochanteric Pain Syndrome (GTPS) is persistent pain on the outside of your hip, caused by irritation of the tissues around the greater trochanter, the bony prominence on the outer part of your upper thigh bone. This condition was previously called trochanteric bursitis, but we now understand it often involves the tendons and muscles around the hip as well as the bursa.

The pain typically presents as a deep ache or tenderness on the outer hip and upper thigh. It often worsens when lying on the affected side at night, climbing stairs, walking long distances, or standing from a seated position. Some people experience pain that radiates down the outer thigh, and stiffness in the hip joint is common, particularly after periods of rest.

Several interconnected factors can contribute to GTPS:

  • Muscle Weakness or Imbalance: Weak hip abductor muscles (particularly the glutes) are the primary culprit. When these muscles can’t stabilise the pelvis properly, excessive stress falls on the tendons and bursa. This is a common sports injury we treat regularly.
  • Overuse and Repetitive Activities: Running, especially on uneven surfaces or with sudden increases in distance, cycling, or prolonged walking can overload the hip structures. Long periods of standing or stair climbing also contribute.
  • Biomechanical Issues: Leg length differences, knee problems, or altered gait patterns place abnormal stress on the hip. Postural syndrome affecting the pelvis and lower back can also play a role.
  • Direct Trauma: A fall onto the hip or a direct blow can trigger the condition, causing immediate inflammation.
  • Age and Gender: GTPS is more common in women over 40, particularly during and after menopause when tendon health can be affected by hormonal changes.

The condition shares similarities with other forms of bursitis, but GTPS specifically affects the hip region and requires targeted treatment to address the unique biomechanics of this area.
